Escalation threshold

The point above which a reconciliation break, a policy exception or a flagged agent action must be escalated to a named human owner rather than resolved automatically. A tolerance without a threshold is a number nobody is accountable for exceeding.

What good looks like

A stated tolerance, a named owner above it, and a record of every time the threshold was actually crossed.
