Proposer–committer split
The design rule that an external agent may propose an action into your systems but never commit it directly — a policy-checked service sits between the two. It is the single control point that keeps an agent integration auditable.
Common causes
A mesh agent given a direct write path to a system of record "to save latency" — removing the one place the organisation could have enforced its policy.
