Threshold policy
The rule stating how many approvals of which roles are required to produce a signature. It is a headcount commitment as much as a control, and it degrades as staff leave unless something is watching the margin.

Set once at design time, never given a live reading, and discovered to be unreachable at the moment a signature is needed.
What good looks like
An alarm at threshold plus one, delegation blocked at the same point, and a rotation that is executed rather than scheduled.
