Delegation chain
The identity-layer record of authority running human → organisation → agent → sub-agent → session, where each link may only carry a subset of the link above it. It matters because it makes "what may this agent do" a checkable question at every hop, including the hops an agent creates for itself, with a person always at the root.
Common causes
A widened child grant is refused with a named reason, never clamped to the parent’s ceiling — because a silent clamp leaves the grant claiming more than it was allowed, live the day the clamp has a bug.
What good looks like
Attenuation at every link, an enforcement endpoint named on every grant, and a root that is a person or an organisation and never a machine.
