Upgrade capture
The condition in which the party able to change deployed contract code is not the party accountable for the system — most often the vendor that built it, holding an administrative key never handed over after delivery.
Common causes
An administrative key retained for operational convenience during delivery, and never moved.
What good looks like
The administrative path held by the accountable institution, under an independent approval set, and exercised at least once on staging.
